About Your Visit

Please arrive 10 to15 minutes before your scheduled appointment time.

Should you need to cancel or reschedule your appointment, please contact our office as soon as possible.

WHAT TO BRING TO YOUR APPOINTMENT

Please bring the following items to your appointment:

  • Insurance Card
    Please bring your insurance card or a copy of your insurance information
  • Patient Forms
    Please print our patient forms packet and complete the forms before your appointment.
  • A List of All Medications
    Please bring a list of all medications you currently are taking or have taken recently, whether over-the-counter or prescription. Include the strength and dosage of each medication.
  • A List of Personal Health Questions or Concerns
    Sometimes we forget all the things we wanted to tell or ask the doctor, so make a list of your symptoms, questions or concerns before you come to your appointment. And don't forget to bring it with you!


WORKERS' COMPENSTATION

If your treatment is covered by Workers' Compensation, please bring the following:

  • Employer's letter of letter of authorization to treat patient
  • Workers' Compensation form completed by your employer
  • Government issued ID card
  • Workers' Compensation Claim #

Insurance

Our office is "in network" with most insurance plans; however, we do not accept Medicaid or Peachstate. The managed care health insurance market changes rapidly, and specific coverage questions should be discussed directly with your insurance company or our knowledgeable staff.
